This review provides a brief overview of Indigenous peoples' issues faced by the mining industry as it seeks to gain access to land, carry out exploration and, if successful, develop and manage a mining operation. It aims to provide guidance on possible options in this area for the International Council on Mining and Metals (ICMM) and the mining industry to contribute to. At the core of this review are results from a survey of ICMM corporate and association members on how they currently define the issues, the challenges they continue to face and the solutions available to them. The results of the industry survey are compared to how other stakeholders define the issues, with priority given to Indigenous peoples and their organizations, but also more generally to development institutions and practitioners, nongovernmental organizations (NGOs), national governments and investors. Learning the proper steps for organizing a failure investigation ensures success. This book outlines a proven systematic approach to failure investigation. It explains the relationship between various failure sources (corrosion, for example) and the organization and conduct of the investigation. It provides a learning platform for engineers from all disciplines: materials, design, manufacturing, quality, and management. The examples in this book focus on the definition of and requirements for a professionally performed failure analysis of a physical object or structure. However, many of the concepts have much greater utility than for investigating the failure of physical objects. For example, the book provides guidance in areas such as learning how to define objectives, negotiating the scope of investigation, examining the physical evidence, and applying general problem-solving techniques.

After introductory material on the history of failure and failure analysis, he provides discussion of such aspects of failure investigation as creation of a database, determination of root cause, timeline of failure analysis, and benefits of a failure investigation. He then outline nine steps of a failure investigation describing the formulation of investigation goals, the identification of possible roots causes, determination of root cause likelihood, and identification and evaluation of corrective action.
