Cohen and his colleagues have gained [practical knowledge of how to improve medication safety through more than 30 years of experience and hundreds of visits to health care organizations, often after a serious accident has occurred. This book attempts to summarize what has been learned from those experiences and to give providers a detailed blueprint for improving medication safety. The book includes much new information gained since the 1999 edition. It should be required reading for anyone interested in or working on improving medication safety. It is now within our grasp to make a huge improvement in medication safety. Doing so will require understanding the magnitude of the problem, studying our medication systems, engaging all the disciplines involved, choosing the right solutions, and building a culture of safety - in sum, a daunting task for any organization. This book provides the tools for that task.
