Written by a team of experts in the field, this book brings the literature up to date and looks at where future developments might be.

The optimal management of bronchospasm and acute asthma is reliant upon confirmation of the diagnosis of asthma, detection of life-threatening complications, recognition of β‎2 agonist toxicity, and exclusion of important asthma mimics (such as vocal cord dysfunction and left ventricular failure). β‎2 agonists, anticholinergics, and corticosteroids are the mainstay of treatment. β‎2 agonists should be preferentially administered by metered dose inhaler via a spacer, and corticosteroids by the oral route, reserving nebulized (and intravenous) salbutamol, as well as intravenous hydrocortisone, for situations when these routes are not possible. A single intravenous dose of magnesium may be of benefit in severe asthma, but repeat dosing is likely to cause serious side effects. Parenteral administration of adrenaline may prevent the need for intubation in the patient in extremis. Aminophylline has an unfavourable side effect profile and has not been shown to offer additional benefit in adults. However, it does have a role in paediatric asthma. Unproven medical therapies with potential benefit include ketamine, heliox, inhalational anaesthetics, and leukotriene antagonists. The need for ventilatory support is usually preceded by worsening dynamic hyperinflation, exhaustion, hypoxia, reduced conscious state, or a combination of these. While non-invasive ventilation may have a temporizing role to allow time for response to medical therapy, there is insufficient evidence for its use, and should not delay invasive ventilation. If invasive ventilation is indicated, a strategy of hypoventilation and permissive hypercapnoea, minimizes barotrauma and dynamic hyperinflation. Extracorporeal support may have a role as a rescue therapy.

We present an effort to port the nonhydrostatic atmosphere dynamical core of the Energy Exascale Earth System Model (E3SM) to efficiently run on a variety of architectures, including conventional CPU, many-core CPU, and GPU. We specifically target cloud-resolving resolutions of 3 km and 1 km. To express on-node parallelism we use the C++ library Kokkos, which allows us to achieve a performance portable code in a largely architecture-independent way. Our C++ implementation is at least as fast as the original Fortran implementation on IBM Power9 and Intel Knights Landing processors, proving that the code refactor did not compromise the efficiency on CPU architectures. On the other hand, when using the GPUs, our implementation is able to achieve 0.97 Simulated Years Per Day, running on the full Summit supercomputer. To the best of our knowledge, this is the most achieved to date by any global atmosphere dynamical core running at such resolutions.

## Background: Data on the efficacy of transcatheter edge-to-edge repair (teer) in patients with cardiogenic shock (cs) are limited. ## Objectives: This study investigated the characteristics and outcomes of consecutive patients with significant mitral regurgitation (mr) and cs who underwent teer. ## Methods: The society of thoracic surgeons/american college of cardiology transcatheter valve therapy registry was assessed from november 22, 2013, to december 31, 2021. cs was defined as the coding of: 1) cs; 2) inotrope use; or 3) mechanical circulatory support before teer. device success was defined as mr reduction of ≥1 grade and a final mr grade ≤2+. the primary outcome was the impact of device success on 1-year mortality or heart failure (hf) readmissions. cox proportional hazards models were used to report the risk-adjusted association between device success and 1-year outcomes. ## Results: A total of 3,797 patients met the inclusion criteria. mean age was 73.0 ± 11.9 years, and 59.5% were male. mean society of thoracic surgery score (mv repair) was 14.9% ± 15.3%. mr etiology was degenerative (53.4%) and functional (27.5%). device success was achieved in 3,249 (85.6%) patients given successful achievement of final mr grade ≤2+ (88.2%) and mr reduction ≥1 absolute grade (91.4%). at 1 year after teer, device success was associated with significantly lower all-cause mortality (34.6% vs 55.5%; adjusted hr: 0.49; 95% ci: 0.41-0.59; p < 0.001) and a composite of mortality or hf admissions (29.6% vs 45.2%; adjusted hr: 0.51; 95% ci: 0.42-0.62; p < 0.001). ## Conclusions: Successful mr reduction is achievable in most patients with cs and is associated with significantly lower mortality and hf hospitalization at 1 year. randomized trials assessing teer in cs are needed to establish this potential therapeutic approach.
