Phantom pain is an intriguing mystery that has captured the imagination of health care providers and the public alike. How is it possible to feel pain in a limb or some other body part that has been surgically removed? Phantom pain develops among people who have lost a limb or a breast or have had internal organs removed. It also occurs in people with totally transected spinal cords. Unfortunately, phantom pain is a medical night mare. Many of the people reporting phantom pain make dispropor tionately heavy use of the medical system because their severe pains are usually not treated successfully. The effect on quality of life can be devas tating. Phantom pain has been reported at least since 1545 (Weir Mitchell as related by Nathanson, 1988) and/ or experienced by such diverse people as Admiral Lord Nelson and Ambroise Pare (Melzack & Wall, 1982; Davis, 1993). The folklore surrounding phantom pain is fascinating and mirrors the concepts about how our bodies work that are in vogue at any particu lar time. Most of the stories relate to phantom limbs and date from the mid-1800s. The typical story goes like this: A man who had his leg ampu tated complained about terrible crawling, twitching feelings in his leg. His friends found out where the leg was buried, dug it up, and found maggots eating it. They burned it, and the pain stopped. The discovery of uniform latex particles by polymer chemists of the Dow Chemical Company nearly 50 years ago opened up new exciting fields for scientists and physicians and established many new biomedical applications. Many in vitro diagnostic tests such as the latex agglutination tests, analytical cell and phagocytosis tests have since become rou tine. They were all developed on the basis of small particles bound to biological active molecules and fluorescent and radioactive markers. Further developments are ongoing, with the focus now shifted to applications of polymer particles in the controlled and di rected transport of drugs in living systems. Four important factors make microspheres interesting for in vivo applications: First, biocompatible polymer particles can be used to transport known amounts of drug and re lease them in a controlled fashion. Second, particles can be made of materials which bio degrade in living organisms without doing any harm. Third, particles with modified surfaces are able to avoid rapid capture by the reticuloendothelial system and therefore en hance their blood circulation time. Fourth, combining particles with specific molecules may allow organ-directed targeting.

These three pioneers labored hard and with unfailing dedication in the early stages of their careers to come up with better ways to trace neuronal connections; they developed tract-tracing methods and used them with such flair as to change forever the course and character of neuroscientific endeavor.The recently deceased Alf Brodal improved the Gudden method for studying retrograde changes in cells following interruption of their efferent fibers . He also became one of the foremost experts in the use of experimental silver methods as witnessed by his many classic investigations of brainstem and cerebellar connections. Walle Nauta was stubbornly convinced of the inherent value of using experimentally induced axonal degeneration in the study of neuronal pathways, and he left no stone unturned in the search for new and better staining solutions. The Nauta silver impregnation method Nauta and Gygax, 1951, 1954) and its various modifications breathed new life into neuroanatomy and were fundamental to the subsequent development of the neurosciences in general. Janos Szentagothai was also a pioneer in the use of experimental silver impregnation techniques ; however, he is more often remembered for the finesse with which he combined light and electron microscopic techniques for the defintion of neuronal pathways and is widely acclaimed for his inspiring and imaginative models of neuronal circuitry.We wish to thank the authors for their contributions and patience during the somewhat lengthy editorial process. Some years ago we, the editors of this volume, found out about each other's deeply rooted interest in the concept of time, the usage of time, and the effects of shortage of time on human thought and behavior. Since then we have fostered the idea of bringing together different perspectives in this area. We are now, there fore, very content that our idea has materialized in the present volume. There is both anecdotal and empirical evidence to suggest that time con straints may affect behavior. Managers and other professional decision makers frequently identify time pressure as a major constraint on their behavior (Isen berg, 1984). Chamberlain and Zika (1990) provide empirical support for this view, showing that complaints of insufficient time are the most frequently report ed everyday minor stressors or hassles for all groups of people except the elderly. This book deals with stars during a short episode before they undergo a ma jor, and fatal, transition. Soon the star will stop releasing nudear energy, it will become a planetary nebula for abrief but poetic moment, and then it will turn into a white dwarf and slowly fade out of sight. Just before this dramatic change begins the star has reached the highest luminosity and the largest diameter in its existence, and while it is a star detectable in galaxies beyond the Local Group, its structure contains already the inconspicuous white dwarf it will become. It is called an "asymptotic giant branch star" or "AGB star". Over the last 30 odd years AGB stars have become a topic of their own although individual members of this dass had already been studied for cen turies without realizing what they were. In the early evolution, so called "E-AGB"-phase, the stars are a bit bluer than, but otherwise very similar to, what are now called red giant branch stars (RGB stars). The inclusion of speakers from non-NATO coun tries provided the opportunity for cross-fertilization of ideas for future research. While the emphasis of the Institute was on basic properties, there was a conscious effort to stimulate interest in the application of metal hydrides to solar/hydrogen energy conver sion schemes in land areas where solar energy has promise as a primary energy source. The need for such tables is acutely felt by all who work in spectroscopy, and numerous requests for such a book have been received by the Commission on Spectroscopy of the Academy of Sciences of the USSR. On the world book market there are fairly complete tables that cover a very great number of spectral lines and that have been complied rather carefully, although they are not free of errors. Tables of this kind are undoubtedly necessary in general spectroscopic research and must be included among the reference books of large scientific institutions. Early Thoughts on Creating Comprehensive Trauma Centers This volume has been many years in writing. When Dr. Donald Meichenbaum first suggested it and I approached my coauthor Lasse Nurmi, it did not seem to be as formidable a task as it has become. Interviewing the centers in this book has taken years-to get responses, to summarize those responses, and to return the summaries for further comment. Many centers have been created in that time; others have suspended operation. This volume does not claim to present even a majority of those centers. However, the ones contained herein are representative of "what is out there. " The idea to create a comprehensive trauma center is not new. The initial section of this forward examines thoughts I proposed as part of my compre hensive examination for my doctorate. Many of the ideas proposed then (1989) seem to fit now. It is my dream to put them into practice someday in the future. THE COMPREHENSIVE EXAMINATION QUESTION In 1989, one question on the written comprehensive examination ques tions for my doctorate was, "If you were to create a comprehensive trauma center in your suburban area, making use of what you have learned in your [doctoral] experience, describe the organization of that center, the mission, structure, personnel, funding, objectives, and services it would offer. Food emulsions have existed since long before people began to process foods for distribution and consumption. Milk, for example, is a natural emulsion/colloid in which a nutritional fat is stabilized by a milk-fat-globule membrane. Early processed foods were developed when people began to explore the art of cuisine. Butter and gravies were early foods used to enhance flavors and aid in cooking. By contrast, food emulsifiers have only recently been recognized for their abil ity to stabilize foods during processing and distribution. As economies of scale emerged, pressures for higher quality and extension of shelf life prodded the de velopment of food emulsifiers and their adjunct technologies. Natural emulsifiers, such as egg and milk proteins and phospholipids, were the first to be generally utilized. Development of technologies for processing oils, such as refining, bleaching, and hydrogenation, led to the design of synthetic food emulsifiers. Formulation of food emulsions has, until recently, been practiced more as an art than a science. The complexity offood systems has been the barrier to funda mental understanding. Scientists have long studied emulsions using pure water, hydrocarbon, and surfactant, but food systems, by contrast, are typically a com plex mixture of carbohydrate, lipid, protein, salts, and acid. Perhaps the most difficult milestone in a young clinician's career is the completion of the first interview. For the typical trainee, the endeavor is fraught with apprehen sion and with some degree of dread. If the interview goes weIl, there is consider able rejoicing; if it goes badly, much consternation results. Irrespective of the amount of preparation that has taken place before the interview, the neophyte will justifiably remain nervous about this endeavor. Thus, the first edition of Diagnostic Interviewing was devoted to providing a clear outline for the student in tackling a large variety of patients in the interview setting. In consideration of the positive response to the first edition of Diagnostic Interviewing, published in 1985, we and our editor at Plenum Press, Eliot Werner, decided that it was time to update the material. However, the basic premise that a book of this nature needs to encompass theoretical rationale, clinical description, and the pragmatics of "how to" once again has been followed. And, as in the case of the first edition, this second edition does not represent the cat's being skinned in yet another way. Quite to the contrary, we still believe that our students truly need to read the material covered herein with considerable care, and once again the book is dedicated to them. We are particularly concerned that in the clinieal education of our graduate students, interviewing has been given short shrift.

In 1970 when I first seriously contemplated writing a book on electron spectroscopy, I recognized the impossibility of completely reaching my desired goals. First, the field was expanding (and still is) at such a rate that a definitive statement of the subject is not possible. The act of following the literature comprehensively and summarizing its essential content proved to be a diver gent series. On the other hand, the field has increased to such a size that violent changes in its basic makeup no longer occur with the frequency that was present in its early days. Furthermore, the excitement of electron spectro scopy lies in its many-faceted interrelationships. In the era of specialization, electron spectroscopy is an open-ended subject continually bringing together new aspects of science. I wished to discuss not just one type of electron spectro scopy, but as many as would be possible. The book as it stands concentrates its attention on x-ray photoelectron spectroscopy, but also presents the basis of Auger electron spectroscopy and uv photoelectron spectroscopy, as well as mentioning many of the other branches of the field. A large, many-author volume might be an answer to some of these problems. However, though anyone person possesses only a limited amount of expertise, I have always enjoyed books by a single author since what they lack in detailed knowledge they gain in a unified viewpoint. Part three presents two case studies as examples of deceptive autonomy and shows the impact of this deception on the situation of women from the viewpoint of cultural studies and social anthropology. Part four relates methodological reflections on feminist and mainstream economics to the theme of the book. The first part of this book is devoted to a reconsideration of Adam Smith as a starting point for feminist perspectives on exchange. Drawing on Adam Smith's Theory of Moral Sentiments Caroline Gerschlager sets the stage for expanding the economic concept of exchange. She analyses and develops Smith's insight that deception is inevitable in the social setting. Smith's system of sympathy, which Gerschlager analyses as a system of exchange, i.e. exchange is conceived in terms of changing places in the imagination, is compared with exchange as conceived by the neoclassical approach. Her analysis reveals that these approaches arrive at contrasting results with regard to deception. Whereas in the former deception is vital to an understanding of exchange, the latter regards deception as an inefficiency, hindering exchange and ultimately making it impossible. The term "carbon-functional organosilicon compound" is used for organosilicon compounds in which a functional group is bonded to an organic moiety that is in turn con nected to silicon via a Si-C bond. Thus, only Si-Cn-Y com pounds (Y designates a functional group) will be discussed in this book 1 Si-O-Cn-Y compounds will in general not be considered, although the latter group does include a large number of natural substances containing silylated hydroxyl groups. (Because of the differing importance of various Y groups, the reader will find some deviation from this restriction). Finally, compounds containing a silyl group as the functional group are not considered. An overview of the field of organosilicon chemistry would show that in the last several decades the commercial synthesis of organosilicon products has increased substan tially, both in annual production and also in the increasing variety of compounds produced. This increase in the number of commercially available carbon-functional monomers and polymers (silicone polymers) is most remarkable and is occurring because new applications are continually being found for these compounds. As might be expected, the number of publications in this field is also increasing. The important position of silicon in the periodic table - between carbon, aluminum, and phosphorus - means that an understanding of the nature of the bonds in organosilicon compounds is quite important in order to understand the bonding in these other areas. This edition of "Ice Cream" is a full revision of previous editions and includes an updating of the areas that have been affected by changes and new technolo gy. The ice cream industry has developed on the basis of an abundant economical supply of ingredients and is a high-volume, highly automated, modern, progressive, very competitive industry composed of large and small businesses manufacturing ice cream and related products. The industry un derwent a difficult period of adjusting to economic changes and to the es tablishment of product specifications and composition regulations. The latter area has now become more stabilized and the Frozen Desserts Definitions and Standards of Identity are now more clearly defined, as are ingredient and nutritional labeling specifications. The chapters that include basic information on ice cream technology remain for the most part unchanged in order to accommodate beginners in the industry and the smaller processors. In other chapters major revisions and the incorporation of new material have been made. Key classical references and information have been retained or added in order to keep intact those portions of the book which students have found most useful and helpful as reflected in my own teaching, research, and publications in the field of dairy science, and particularly in the field of ice cream production. It has been sixteen years since the unification of electro Magnetism with the weak interactions was developed by Glashow, Salam, and Weinberg. Well before that proposal was fully confirmed by experiment, work began on unifying strong interactions with the electroweak. Now there is a growing effort to incorporate some theory of quantum gravity into the scheme. This enormous complex of theoreti cal and experimental efforts was the subject of the Fourth Workshop on Grand Unification held in Philadelphia and attended by over two hundred physicists. During the workshop, experimental and theoretical talks alternated as shown by the program summary on page 409. However, to display the logical scope of the workshop the proceedings are organized into five subject areas. Howard Georgi opened the workshop with a keynote address in which he reminds us of some of the simple properties of the particle spectrum that have not yet been understood. The first subject area, and also the largest, is proton decay and underground physics. Birch reduction (see reviews [1-5]) is the name given to the reaction of unsaturated organic compounds with alkali metals and alcohols in liquid am monia. This method was first used for aromatic compounds in 1937 by Wooster [6J, who showed that benzene and its derivatives are reduced by sodium in liq uid ammonia in the presence of an alcohol, while this reaction does not take place in the absence of an alcohol. However, the general recognition and broad application of this reaction was achieved only after a series of investigations by Birch published from 1944 onwards [7J. In the years since its development in West Gennany and particularly since its arrival in the United States, extracorporeal shock wave lithotripsy (ESWL') has become the treatment of choice for most cases of urinary lithiasis. The fIrst shock wave lithotripsy patient in the U.S. was treated with a Domier HM3 in February of 1984 at the Methodist Hospital of Indiana. In response to the great enthusiasm generated by this new treatment modality, the following year the MHI presented its fIrst symposium on shock wave lithotripsy. Each year the meeting generated more and more interest. Following the 1988 symposium, the presentations were published in a book entitled Shock Wave Lithotripsy: State of the Art. Following on the heels of the success of kidney stone treatment with ESWL, the new field of biliary lithotripsy rapidly was gaining momentum. In response to the great interest generated by this additional application of the technology, the 1989 meeting focused special attention on this new method of treating gallstones. Methodist Hospital's 5th Symposium on Shock Wave Lithotripsy, which was presented in March of 1989, had the largest attendance ever, with over 600 physicians from 42 states and 20 countries. The publication of the proceedings of this meeting was made possible by generous educational grants from Domier Medical Systems, Inc., Marietta, Georgia, and the Methodist Hospital of Indiana. Masters Theses in the Pure and Applied Sciences was first conceived, published, and dis seminated by the Center for Information and Numerical Data Analysis and Synthesis (CINDAS) * at Purdue University in 1957, starting its coverage of theses with the academic year 1955. Beginning with Volume 13, the printing and dissemination phases of the ac tivity were transferred to University Microfilms/Xerox of Ann Arbor, Michigan, with the thought that such an arrangement would be more beneficial to the academic and general scientific and technical community. After five years of this joint undertaking we had concluded that it was in the interest of all concerned if the printing and distribution of the volume were handled by an international publishing house to assure improved service and broader dissemination. Hence, starting with Volume 18, Masters Theses in the Pure and Applied Sciences has been disseminated on a worldwide basis by Plenum Publishing Corporation of New York, and in the same year the coverage was broadened to include Canadian universities. All back issues can also be ordered from Plenum. We have reported in Volume 24 (thesis year 1979) a total of 10,033 theses titles from 26 Canadian and 215 United States universities. We are sure that this broader base for theses titles reported will greatly enhance the value of this important annual reference work.
